- Make of you vehicle (Polaris, John Deeres, etc)
- Model of your vehicle (RZR, RANGER, etc)
- Vehicle model year (2019, 2005, etc)
- Number of Seats
- Tire Size
- Serial number on the primary of the DURACLUTCH kit
- Odometer reading
- VIN number
- Modifications to the vehicle
- Where your Duraclutch kit was purchased
- When your Duraclutch kit was purchased
- Date your Duraclutch kit was installed
